Given that I know very little about your motivations of switching from physician to MBA, it's hard for me to comment on your chances of admission. Harvard and Stanford are incredibly competitive schools and you will need to show a compelling reason as to why they are good matches for your career interests. They will be receptive to a career-changing doctor, but only if you've shown you've thought this decision through.
You conceivably can apply before you take your GMATs but your application won't be classified as "complete" until the school sees that GMAT results.
