I think that "William Pereira first gained national recognition for his movie set designs, including that for the 1942 film “Reap the Wild Wind,”", should be the correct answer, because "that" is referring to "the 1942 film “Reap the Wild Wind,”" and not to the "movie set designs". If we take the sentence as it is, then it may mean "William gained multiple recognition's for the 1942 film “Reap the Wild Wind,"".

Please correct me if wrong.

If, in fact, you're right, then we should be able to replace "that" with "the 1942 film 'Reap the Wild Wind.'" Let's try it:

William Pereira first gained national recognition for his movie set designs, including that for the 1942 film “Reap the Wild Wind,
becomes
William Pereira first gained national recognition for his movie set designs, including the 1942 film 'Reap the Wild Wind' for the 1942 film “Reap the Wild Wind,

The correct antecedent is "movie set designs."

On Similar lines why cannot 'that' refer to 'national recognition' ?

William Pereira first gained national recognition for his movie set designs, including the 'national recognition' for the 1942 film “Reap the Wild Wind, ....

There's a change in meaning. He does not get national recognition for the films, he gets it for the set designs. That's why "that" can't be an antecedent for "national recognition."

I hope that makes sense.
